2025 Open Enrollment

This is your annual opportunity to make changes to your medical and vision plan, enroll/re-enroll into FSA and/or AFLAC Advantage Plans without having a qualifying event. Enrollments or changes made during Open Enrollment will be effective January 1, 2025.

2025 Health & Vision Benefits

Health and Vision Open Enrollment is from September 30 to October 25, 2024 5 p.m. PST.

Please review your current medical/vision plan and other medical plans offered to you. If you want to change your medical/vision plan or want to know more about next year’s benefits, visit at or call SFHSS at 628-652-4700.

Note: If you are currently enrolled in the VSP Premier Plan, you and your dependents will automatically be re-enrolled in the VSP Premier Plan next year. If you do not wish to re-enroll in VSP Premier, you need to elect VSP Basic Plan through eBenefits or 2025 Enrollment Form.

Enroll or make medical/vision plan changes through eBenefits self-service. If you are unable to enroll online through eBenefits, you can submit your 2025 Open Enrollment Form and any required verification documentation directly to SFHSS through one of the following methods:

  • Fax to SFHSS at 628-652-4701
  • Mail to SFHSS 1145 Market Street, 3rd Floor, SF, CA 94103
  • Drop off using the SFHSS Drop Box located at 1145 Market Street, 3rd Floor, SF, CA 94103 from 9:00 a.m. to 4:30 p.m.

Note: Keep a copy of your form and fax confirmation if faxed.

In December, SFHSS will send a 2025 enrollment confirmation letter to your address on file. Please make sure to review your Open Enrollment material. If you have any questions, please call SFHSS directly at 628-652-4700.

Flexible Spending Accounts (FSA): Health Care & Dependent Care

FSA Open Enrollment is from October 1 to November 30, 2024 5 p.m. PST.

**All current enrollees MUST re-enroll for the 2025 plan year to continue FSA Healthcare and/or FSA Dependent Care. Enrollment is for the plan year starting on January 1, 2025.

Note: Part Time Faculty and Classified employees working less than 20 hours/week and As-Needed are not eligible for FSA.

Flexible Spending Accounts are voluntary plans. There are two different types of Flexible Spending Accounts (FSA): FSA Health Care and FSA Dependent Care. The FSA accounts allows you to set aside pre-tax dollars for qualified expenses incurred by you, your legal spouse, dependent(s), or relative(s) (as defined in “Internal Revenue Code Section 125, which excludes certified domestic partners).

To calculate your annual election, you need to determine the annual contribution you want to make to your FSA Health Care account (i.e. health, vision or dental care, etc.) for you and your dependents, and/or FSA Dependent Care account for daycare expenses. Children must be under the age of 13. FSA Dependent Care cannot be used for dependent medical, dental or vision expenses.

Keep in mind that you cannot transfer money between the two accounts, and you cannot transfer participation to your spouse. Only expenses incurred on or after your effective date of coverage are reimbursable.

Unused 2025 FSA Healthcare, up to the maximum carryover fund amounts, can be carried over to the following year. The carryover amount will be determined at the end of the claim filing period (March 31, 2026). 2025 Carryover funds can only be accessed for one plan year (December 31, 2026) and any remaining carryover funds will be forfeited. You do not need to enroll in 2025 FSA to have 2024 carryover in 2025 plan year.

Unused FSA Dependent Care funds are forfeited at the end of the plan year and cannot be returned to you.

In accordance to district policy, your annual election will be divided by the number of pay periods (or remaining number of pay periods) according to your classification.

AFLAC plans are portable beyond ÃÈ·­µ¼º½ employment. Ask about AFLAC Always.

Employees and eligible dependents who are enrolled into a medical plan with San Francisco Health Service System (SFHSS), will automatically be enrolled into the VSP Basic Vision Plan. Employee and dependents enrolled in a medical plan with SFHSS may elect the Premier VSP Plan.

VSP does not issue insurance cards. If you are enrolled into the VSP plan (group# 12145878), please call VSP at 1-800-877-7195 for your member ID number.

If you have a qualifying event during the year, you can add or drop your dependents from your benefits. You have within 30 days from your qualifying event to make changes to your benefits.

How to Calculate Domestic Partner (DP) Premium

  • For Employee + 1 DP: Employee + 1 subtract Employee Only premium
  • For Employee + 1 DP + 1 Dependent: Employee + 2 or more subtract Employee + 1 premium
  • For Employee + 2 DP or more: Employee + 2 or more subtract EE only premium

*** The DP premium is generally post tax unless you are qualified for the Federal Tax Exemption for Dependents Who Meet Certain Requirements or Equitable California State Tax Treatment and complete the SFHSS Domestic Partner Declaration Form. Unused Flexible Spending Account Health Care dollars up to $660 can be carryover to the following year 2026.  

Dependent Care FSA is a pre-tax benefit account used to pay for dependent care services, such as preschool, summer day camp or elder daycare. Dependent Care FSA minimum annual contribution is $50 minimum and the maximum contribution is $5,000 per calendar year ($2,500 if married and filing a separate tax return).

Basic Life and Accidental Death and Dismemberment (AD&D) is offered to eligible employees at no premium cost. Coverage amount is $50,000 (under age 70) and coverage will reduce on the 70th and 75th birthday.

Beneficiar(ies) can be updated at any time by completing a Life Insurance Enrollment form. It is important to name beneficiaries to ensure that any benefits payable at your death are left to survivors you intended.

EAP provides confidential, voluntary, free counseling services to employees. Get one-on-one counseling for personal and work-related concerns form a licensed EAP therapist. Active and eligible employees have access to 6 counseling sessions for each issue in a 12-month period.
